Wall Residences has two Admissions Coordinators covering the state of Virginia, Pam Golas and Alex Jackson.  Each is responsible for specific regions, ensuring that individuals desiring services are offered appropriate choices of potential Providers in whatever area they wish to live.

 

 

 

Referrals can be made by Support Coordinators/Case Managers, Guardians, family members, and/or an individual by phone or email to Pam or Alex.  At that time the following documents, as available, will be requested: Current Psychological Evaluation, Current Level of Functioning Survey, Social Assessment, Current Physical/List of Medications, SIS (when applicable) and Current Shared Plan/Individual Support Plan.  Contact information for all involved people in the individual’s life will also be gathered.  In cases of an emergency referral, which Wall Residences is able to accommodate, this information will need to be submitted as soon as possible.

 

 

 

Pam and/or Alex will schedule a time to meet with the individual and others who know that person well (parents, family members, guardians, staff in current placement, etc.) to determine that person’s dreams and desires, in addition to support needs, in order to find the right match in potential providers.  This is done through an extensive interview process and observation.  As Wall Residences has many talented Providers with the capacity to support individuals who may experience significant challenges, this information is used to develop a unique service for the referral. Matching the individual to the Provider that can best meet these needs is critical for the long term success of the service.  Assessment tools (ICAP, Personal Profile, Fall Risk, Capacity evaluation, etc.) can also be completed during the initial meeting and/or during subsequent meetings.

 

 

 

Pam and Alex are acquainted with vacancies in their respective regions and will contact Providers in the area identified as where the individual wishes to live to share information in order to offer choice to the individual.  Often there is more than 1 Provider service in which the individual might be interested, and a tour of each home will be scheduled by the appropriate Admissions Coordinator.    At this point in the admissions process, the Regional Program Manager for the service (s) will become involved and take over from the Admissions Coordinator; the individual and/or their family, guardian or other decision maker, along with the chosen Provider will decide after a trial visit if this is the right service for long term success.

 

 

 

Pam and Alex are available for participation in Provider Fairs in addition to speaking to Support Coordinators, Parents Groups and others interested in learning more about Sponsored Residential Services and Wall Residences.
